Here are some of the most common forms that I may ask you to print off and fill out ahead of time and bring to your first appointment should you decide to schedule one. Filling these out at your own leisure at home or at your place of employment will conserve more of your appointment time for your opportunity to discuss the reasons you are currently seeking counseling.

Should you be unsure how to complete these forms, feel free to call me or leave blanks and ask about them during your first session. There will be plenty of time for you to ask any questions you may have. It may be helpful for you to write your questions down ahead of time so you don’t forget to ask what is most important to you.

Especially if this is your very first time in therapy, you may be a little anxious and not know what to expect. Under such circumstances it is easy to forget things even if they are very important to you.
